Form FTB 3519 is the payment voucher California residents and nonresidents use to send money to the Franchise Tax Board when they need extra time to file their state income tax return but still owe a balance. California automatically grants a six-month extension to file, so you don’t need to submit a separate request for the extension itself. What you do need is a way to pay any tax due by the original deadline, and that’s exactly what this form covers.

What Form FTB 3519 Actually Does

Unlike an IRS extension request, California doesn’t require paperwork just to get more time to file. The extension is automatic. However, that extra time to file is not extra time to pay. If you owe state income tax, the FTB still expects payment by the regular deadline (typically April 15). Form 3519 is simply the voucher that accompanies your payment, whether you’re mailing a check or need a record of the amount you’re submitting electronically.

The form includes fields for your name, Social Security number, address, and the payment amount, all of which are fillable digitally since it’s built as an AcroForm PDF. That means you can type your information directly into the file before printing it, rather than filling it out by hand.

Who Needs to File This Form

You’ll use Form FTB 3519 if you’re an individual taxpayer in California who expects to owe state tax but can’t finish your return by the deadline. This applies whether you’re a full-year resident, a part-year resident, or a nonresident with California-source income. Business entities have their own separate extension payment vouchers, so this one is specifically for individual filers.

If you’ve already paid enough through withholding or estimated payments and don’t owe anything additional, you generally don’t need to submit this voucher at all. It’s only necessary when there’s a balance due that needs to accompany the extension period.

When to Use Form 3519

Timing matters here. The payment associated with Form 3519 is due on the original filing deadline, not the extended one. So even though California gives you until October 15 to actually file your return, your payment obligation doesn’t move. Missing that April deadline means you’ll likely face penalties and interest on the unpaid balance, even if your return itself is filed within the automatic extension window.

This is a common point of confusion for taxpayers who assume an extension to file also means an extension to pay. It doesn’t, at the state or federal level, and California is no exception.

How to Get and Fill Out the Form

The official 2025 version of Form FTB 3519 is available directly from the Franchise Tax Board’s website as a downloadable PDF. Because it’s a fillable AcroForm, you can open it in most PDF readers, enter your details on-screen, and then print the completed voucher to mail alongside your check or money order. The FTB also allows electronic payment options through their online portal, in which case you may not need to mail a physical form, but keeping a copy for your records is still a good idea.

When filling it out, double-check your Social Security number and the payment amount, since errors here can delay processing or cause the payment to be misapplied. If you’re also dealing with tax credits or specialized filings in another state, it’s worth comparing how those systems handle extensions and payments differently. For instance, taxpayers juggling multiple state obligations sometimes look into industry-specific credits like the Investment Tax Credit for the Financial Services Industry or alternative energy incentives such as the Claim for Alternative Fuels Credit, both of which follow entirely different filing rules than California’s extension process.

Once you’ve submitted Form 3519 and your payment, there’s nothing further to file for the extension itself. You simply proceed to complete your full California return by the extended October deadline, referencing the payment you already made so it’s properly credited against your final tax liability.
